- Étude menée à Hong Kong au Yan Chai Hospital - Hong Kong Baptist University Chinese Medicine Clinic cum Training and Research Centre.
Patients
- Patients avec cancer colorectal métastasés et espérance de vie > 6 mois.
- Chimiothérapie programmé par oxaliplatine .
Interventions
60 patients sont randomisés en deux groupes :
- Acupuncture (figure 1 et 2).
- Fausse acupuncture. Utilisation d’aiguilles d’acupuncture placebo de Streitberger (figure 3) au niveau des mêmes points avec une fausse électrostimulation. Les autres paramètres sont similaires.
Principaux critères de jugement
- Échelle de neurotoxicité FACT/GOG-Ntx.
- Qualité de vie [QLQ-C30].
Pincipaux résultats
- Diminution significative de la sévérité de la neuropathie à l'échelle FACT/GOG-NTX durant le 2ème cycle, mais non à la semaine 12 (fin du traitement) ou à la semaine 24 (fin du suivi) [figure 4].
- Amélioration significative de la qualité de vie dans la sous-échelle fonction physique durant le 3ème cycle et fonction sociale durant le 2ème cycle, mais non sur l'échelle globale ou les différentes sous-échelles à la semaine 12 ou à la semaine 24 ou aux autres différentes temps d'évaluation.

Conclusion des auteurs
Mise en évidence de preuves préliminaires sur l'efficacité de l'acupuncture dans la neuropathie chimio-induite aigue et non chronique chez les patients avec cancer colorectal métastasé.
Commentaires
Les spins
- Les auteurs mettent en avant un résultat positif lors du 2ème cycle de chimiothérapie, mais les résultats sont non significatifs à tous les autres temps de l'étude. Parler de "preuves préliminaire" sur la neuropathie aigue est une manipulation de langage, un "spin de conclusion".
- On parle de "spins de conclusion" (ou enjolivement de la conclusion) quand la conclusion d'un article rapportant un essai clinique est positive, en faveur de l’intérêt du traitement, alors même que l’essai est non concluant. C'est une stratégie d'expression des résultats visant à mettre en avant un bénéfice du traitement évalué.
- La question des spins n'est pas particulière à l'acupuncture, mais concerne toute la médecine. On retrouve ainsi des spins dans 62% des études en orthodontie [2], 56% en psychiatrie [3], 44% en chirurgie orthopédique [4], 34% en médecine d'urgence [5].
L'acupuncture dans les neuropathies chimio-induites
- On parle de neuropathie périphérique chimio-induite (NPCI) aigue quand elle survient durant la chimiothérapie (85 à 90 % des patients sous oxaliplatine) et chronique quand elle persiste après la fin de la chimiothérapie (30 à 50 % des patients sous oxaliplatine).
- L'essai rapporté est à faible effectif et est négatif. On peut éventuellement évoquer une puissance statistique insuffisante, d'autant plus que la fausse acupuncture utilisée est montrée avoir un puissant effet placebo [6, 7].
- Le résultat de l'essai est discordant avec les revues systématiques récentes (2022-2023) qui concluent à un intérêt de l'acupuncture dans cette indication [8-11].
- L'acupuncture figure dans les options thérapeutiques des dernières recommandations de bonne pratique publiées : américaines (2022 [12]), allemandes (2021 [13], ou internationales (2022 [14]. Il faut noter que les précédents guidelines américains et allemands (2020) des mêmes institutions étaient négatif, témoignant de l'évolution générale des données concernant l'acupuncture.
- Le protocole d'acupuncture utilisé dans l'étude apparait comme pertinent. Une revue montre que les points les plus utilisés dans les essais cliniques dans cette indication sont : 4GI, 11GI, 36E, bafeng et baxie [15].
